Điều 1. Điều 1. Nay ban hành kèm theo Quyết định này "Mẫu giấy chứng nhận kiểm dịch thực vật" . Mẫu giấy này thay thế cho mẫu số 9 trong Quy định ban hành kèm theo Quyết định số 91/1998/QĐ-BNN-BVTV ngày 02 tháng 7 năm 1998 của Bộ trưởng Bộ Nông nghiệp và phát triển nông thôn.
Điều 2. Điều 2. Quyết định này có hiệu lực sau 15 ngày kể từ ngày ký. Những quy định trước đây trái với Quyết định này đều bãi bỏ.
Điều 3. Điều 3. Ông Chánh Văn phòng Bộ, Cục trưởng Cục Bảo vệ thực vật, Thủ trưởng các cơ quan liên quan chịu trách nhiệm thi hành Quyết định này.
